Parents of private/parochial school students are advised that they must submit a signed Transportation Request Form for new or continued bus service.
The Transportation Office must receive this form on or before April 1 for the following new school year. For those who move into the Sag Harbor School District after April 1, a signed Transportation Request Form must be filed within 30 days of moving into the district.
Please be advised that Transportation Request Forms are provided by the private/parochial schools only. The completed Transportation Request Form may be mailed or faxed to the School District to:
